Paracentral lobule
The paracentral lobule is the region on the medial surface of the cerebral hemisphere where the precentral and postcentral gyri continue over the midline. It contains the motor and sensory representation of the contralateral lower limb and perineum.
The paracentral lobule is a U-shaped region of cortex on the medial surface of each cerebral hemisphere, straddling the point where the central sulcus reaches the top of the brain. It is the medial continuation of two landmark gyri from the lateral surface: the precentral gyrus (primary motor cortex) in front of the central sulcus and the postcentral gyrus (primary somatosensory cortex) behind it.
Its functional importance follows from the motor and sensory homunculi. Because the body map runs from the face laterally up to the leg medially, the cortical territory for the contralateral lower extremity and perineum folds over the top of the hemisphere into the paracentral lobule. The anterior portion handles voluntary movement of the opposite leg and foot, the posterior portion processes its sensation, and the region also contributes cortical control of micturition and defecation.
The clinical correlate is vascular. The medial hemisphere, including the paracentral lobule, is supplied by the anterior cerebral artery (ACA) — not the middle cerebral artery that serves the lateral face and arm areas. An ACA stroke therefore classically produces contralateral weakness and sensory loss that is worse in the leg than the arm or face, sometimes with urinary incontinence from loss of cortical bladder control. That lesion pattern is the reverse of an MCA stroke and is a favorite exam discriminator.
On USMLE Step 1, the paracentral lobule appears in neuroanatomy questions that pair a lesion or vascular territory with a deficit: expect vignettes of leg-predominant hemiparesis or new incontinence and be ready to localize to the medial frontal/parietal cortex and the ACA.
