Types of study design

Also known as: research study designs, epidemiologic study designs

Types of study design are the frameworks researchers use to investigate health questions, divided into observational designs — case reports, cross-sectional, case-control, and cohort studies — and experimental designs like randomized controlled trials.

Study designs are the blueprints of medical research, and they divide into two families. In observational studies, researchers watch what happens without intervening; in experimental studies, researchers assign an intervention and measure its effect. The design determines what kinds of conclusions a study can support and which biases threaten it.

Observational designs form a ladder of increasing strength. Case reports and case series describe individual patients with no comparison group. Cross-sectional studies measure exposure and outcome at a single point in time, yielding prevalence but not causation. Case-control studies start with people who have the disease (cases) and without it (controls), then look backward at exposures — efficient for rare diseases, and analyzed with the odds ratio. Cohort studies follow exposed and unexposed groups forward (or reconstruct them retrospectively) to see who develops disease, yielding incidence and the relative risk.

The randomized controlled trial (RCT) is the experimental gold standard: random assignment balances known and unknown confounders between treatment and control groups, and blinding limits measurement bias. Above individual studies sit systematic reviews and meta-analyses, which pool results across trials and top the hierarchy of evidence.
