Cerebellum and brainstem

The cerebellum is the hindbrain structure that coordinates movement, balance, and motor learning, while the brainstem — midbrain, pons, and medulla — relays signals between the brain and spinal cord and controls vital autonomic functions such as breathing and heart rate.

The brainstem consists of three segments. The midbrain contains the cerebral peduncles, the substantia nigra, and the nuclei of cranial nerves III and IV, and its colliculi mediate visual and auditory reflexes. The pons carries the middle cerebellar peduncles and the nuclei of cranial nerves V through VIII, and contributes to respiratory rhythm. The medulla contains cranial nerve nuclei IX through XII, the cardiovascular and respiratory centers, and the pyramids where the corticospinal tract decussates. Because ascending sensory and descending motor tracts all pass through this narrow corridor, small brainstem lesions can produce large and distinctive deficits.

The cerebellum sits behind the brainstem and connects to it through three pairs of peduncles. It does not initiate movement; it compares the motor command with sensory feedback about what the body actually did and issues corrections. Functionally it divides into the vestibulocerebellum, which governs balance and eye movements; the spinocerebellum, which controls posture and limb coordination; and the cerebrocerebellum, which supports planning and timing of skilled movement.

Localization follows from that anatomy. Cerebellar lesions cause ipsilateral deficits — intention tremor, dysmetria, dysdiadochokinesia, ataxic gait, and nystagmus — because the cerebellar output pathway crosses twice. Vermis and flocculonodular lesions preferentially affect truncal stability, while lateral hemisphere lesions affect the limbs. Brainstem strokes produce crossed findings: ipsilateral cranial nerve deficits with contralateral body weakness or sensory loss, the pattern seen in syndromes such as lateral medullary (Wallenberg) stroke.

USMLE Step 1 tests this material as clinical localization. Given a set of findings, you should be able to name the affected structure and level, predict the deficits from a described lesion, and trace the vestibular and auditory pathways that pass through the pons and medulla.
