Lymphatic system

Also known as: lymphatic and immune system

The lymphatic system is a network of vessels, nodes, and organs that returns excess interstitial fluid to the bloodstream, absorbs dietary fats from the intestine, and houses the immune cells that screen that fluid for pathogens.

Lymphatic capillaries begin as blind-ended tubes woven through nearly every tissue. Their overlapping endothelial cells act as one-way flaps, admitting the fluid, protein, and cellular debris that leak out of blood capillaries. Once inside, the fluid is called lymph. It travels through progressively larger vessels and lymph nodes into lymphatic trunks, then into the thoracic duct or the right lymphatic duct, and finally back into the venous circulation at the junction of the internal jugular and subclavian veins.

Drainage is regionally divided. The right lymphatic duct clears the right side of the head, neck, and thorax and the right upper limb; the thoracic duct, which begins at the cisterna chyli, drains everything else. There is no central pump — lymph moves through valved vessels propelled by skeletal muscle contraction, arterial pulsation, and pressure changes from breathing, which is why immobility and vessel obstruction lead to swelling. Lymphoid organs sit along the route: lymph nodes filter lymph, the spleen filters blood, the thymus matures T lymphocytes, and tonsils and Peyer's patches guard mucosal surfaces.

Three functions define the system. It maintains fluid balance by returning several liters of filtered plasma to the circulation each day; blocked drainage causes lymphedema. It absorbs dietary fat through specialized lacteals in the small intestine, which carry chylomicrons as milky chyle into the thoracic duct. And it provides immune surveillance, concentrating antigen in nodes where lymphocytes and antigen-presenting cells can meet — the reason nodes enlarge and become tender during infection.

Coverage varies by exam. The MBLEx tests lymphatic anatomy and the drainage direction that guides massage technique and contraindications; the CCMA exam covers lymphatic and immune disorders in a clinical assisting context; and the MCAT treats the lymphatic system as part of organ system structure and function alongside the immune response.
