Narrow therapeutic index (NTI) medications

Narrow therapeutic index medications are drugs whose effective dose is very close to the dose that causes toxicity. Small changes in dose, formulation, or absorption can push a patient from therapeutic benefit into harm, so these drugs require careful monitoring.

The therapeutic index is the ratio between the dose that produces toxicity and the dose that produces the desired effect, often written as TD50 / ED50. A large ratio means a wide safety cushion; a small ratio means the therapeutic window is narrow. For an NTI medication, the difference between a subtherapeutic level, an effective level, and a toxic level may be only a small percentage change in blood concentration.

Commonly cited NTI drugs include warfarin, digoxin, lithium, levothyroxine, phenytoin, carbamazepine, valproic acid, theophylline, cyclosporine and tacrolimus, and antiarrhythmics such as procainamide. Because the margin is thin, therapy is guided by measured serum concentrations rather than dose alone — INR monitoring for warfarin, serum digoxin and lithium levels, free phenytoin levels in patients with low albumin, and trough levels for immunosuppressants. Dose adjustments are made in small increments.

Several factors can shift a patient out of the window without any change in the prescription. Drug interactions that induce or inhibit metabolizing enzymes, declining renal or hepatic function, dehydration, changes in protein binding, and even a switch between formulations can alter serum concentration meaningfully. For that reason many states restrict generic substitution of NTI drugs without prescriber notification or consent, and pharmacies flag these products for extra verification.

The PTCE tests NTI medications directly, expecting technicians to recognize the drug list, understand why substitution and dispensing require extra care, and know that these products often carry monitoring requirements.
