Physical incompatibility (pharmacy)

Physical incompatibility occurs when two or more drugs or ingredients mixed together produce a visible physical change — such as precipitation, cloudiness, color change, or separation — without necessarily reacting chemically.

Physical incompatibility is one of the three classic types of pharmaceutical incompatibility. It occurs when combining drugs or excipients produces an unacceptable physical change in the preparation: the ingredients fail to blend into a uniform, stable product, even though a true chemical reaction may not have occurred.

The telltale signs are usually visible. Common examples include precipitation (a dissolved drug crashing out of solution, as when a drug formulated for acidic solution is added to an alkaline one), turbidity or cloudiness, haze, color change, gas formation, and phase separation such as an emulsion cracking or oily layers splitting. Insolubility problems — mixing a water-insoluble drug into an aqueous vehicle, or exceeding a drug's solubility limit — are classic physical incompatibilities, as is liquefaction of certain solid ingredients when triturated together (eutectic mixtures).

Recognizing physical incompatibility matters most in IV admixture and compounding. A precipitate in an intravenous line can obstruct the catheter or embolize, which is why certain drug pairs must never be mixed in the same syringe or Y-site, and why lines are flushed between medications. Physical incompatibility is distinguished from chemical incompatibility, where ingredients degrade through reactions like oxidation or hydrolysis (often invisibly), and therapeutic incompatibility, where drugs interact pharmacologically within the patient.

The PTCE tests pharmaceutical incompatibilities within its medications domain. Pharmacy technician candidates should be able to classify an incompatibility as physical, chemical, or therapeutic, and recognize visible signs — precipitation, cloudiness, separation — that mean a compounded or admixed product must not be dispensed.
