Pharmaceutical incompatibilities are unwanted reactions that occur when two or more drugs or excipients are mixed, compounded, or administered together, compromising the product's safety, effectiveness, or appearance. They differ from drug-drug interactions inside the body: an incompatibility happens outside the patient — in the syringe, IV bag, or compounding vessel — before the medication is ever absorbed.

Physical incompatibilities change the preparation's physical state without a chemical reaction: precipitation of an insoluble drug, immiscibility of oil and water phases, adsorption of a drug onto a container, or liquefaction of solids mixed together. The result is often visible — cloudiness, particles, phase separation.

Chemical incompatibilities involve actual reactions between ingredients — oxidation, hydrolysis, or acid-base reactions that inactivate a drug or create degradation products. A well-known example is mixing calcium-containing solutions with phosphate or with ceftriaxone, which forms dangerous precipitates in IV lines. Therapeutic incompatibilities occur when combined drugs produce an unintended pharmacologic result, such as antagonistic effects that cancel each other out. Pharmacy staff prevent incompatibilities by checking compatibility references, flushing IV lines between drugs, using separate lines, and observing admixtures for visible changes.
